Form Settings
When you create a form, you must specify how you wish to be notified when a visitor submits a form. The email address is the only required field on this page.
1 In the Email address text box, enter as many as 5 email addresses where the form results will be sent.
2 In Email subject, enter a subject line for the email or keep the default text that displays.
3 In Email format, choose to receive the results in Human readable or Comma separated format. If you are unsure, we recommend Human readable format.
4 In Response Message, enter the message that visitors will see when they submit a form to you.
5 HTML Mode allows you to make changes to the form using HTML. This is an advanced setting that requires HTML knowledge. If you are not experienced with HTML, we recommend that you do not use this setting.

6 If you would like to select your own method of processing form submittals, click the Advanced Options button.

7 Click the "Done" button to add the new form and return to the page.
