In the Name field, enter an attribute name that will be used to identify this form element. This name is required, but is not visible within the form.

In Display label, you can enter the text that appears beside the checkbox.

Submit value, is an attribute that appears within the submission that is returned to you, but does not appear as part of the form.

Default value, allows you to choose whether the box is checked by default on the form. Put a checkmark in this box to make the box checked by default.

HTML Mode brings you to an advanced screen where you can customize the checkbox using HTML tags. Note that this feature requires knowledge of HTML.
